Matching State Forms is the Key!
The insurance application is generally taken in the policy owner's state of residence. However, another state's application may be used if the policy owner is employed, owns a business or has a second home in another state.

PLEASE REMEMBER
The state used for the Application Part A must also be used for any accompanying state-specific forms.

Keep in mind:

The paramedical exam itself can be performed in and signed in any state. However, the Application Part B state must match the Application Part A state.

For AGL policies where the policy owner is a New York state resident applying in another state, a Request for Cross-Border Exception on a New York Resident (AGLC110219) is required.

For trust-owned policies, applications must use the state where the trust is domiciled. New York-domiciled trusts may only apply for USL policies.
